I understand and acknowledge that the spa facilities at The Davenport Hotel are self-service and unsupervised by staff. The available amenities may include, but are not limited to: sauna, steam room, vitality pool, swimming pool, relaxation lounges, showers and changing rooms and plunge pool. I confirm that I am voluntarily choosing to use the spa facilities at my own risk, and that I am physically able and medically fit to do so.
I confirm that I do not suffer from any medical condition that would make the use of heat, steam, or hydrotherapy dangerous, including but not limited to: heart conditions, high or low blood pressure, respiratory issues, pregnancy, epilepsy or seizures, skin infections or open wounds, etc. If I have a condition or concern, I will consult with my doctor prior to using the facilities. I agree to follow all posted rules and spa etiquette at all times. I understand that failure to follow said rules may result in removal from the spa without refund.
I hereby waive, release, and discharge The Davenport Hotel, its employees, owners, and affiliates from any and all claims, damages, or causes of action arising from my use of the spa facilities, except in the case of proven gross negligence or wilful misconduct on the part of the hotel.
I understand that The Davenport Hotel is not responsible for: loss or damage to personal belongings, personal injury or illness resulting from spa use, medical costs or emergency care associated with spa use. Any personal data collected in this form will be used solely for the purpose of safety, record-keeping, and guest services, in accordance with GDPR regulations. Your data will not be shared with third parties and will be securely stored.
